A limited number of forms and instructions are available at the library. If you need a form we do not have, we will print a copy for you for free.
Federal
We have the following forms and instructions:

Unemployment Insurance Tax Information
Unemployment insurance benefits are considered taxable income. If you collected unemployment benefits you will need to complete a 1099-G tax form. See the Wisconsin Department of Workforce Development 1099-G Tax Information webpage for information about the 1099-G form.
